All visits are conducted by Jerleshia Fields,APRN, FNP-BC, PMHNP-BC a Florida-licensed, board-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner with over 23 years of healthcare experience and 14 years of experience as a Nurse Practitioner.

Telehealth services may be appropriate for many non-emergency conditions, including:
Minor acute illnesses
Chronic condition management

Certain conditions may require in-person evaluation or referral.

Yes. Medications may be prescribed when clinically appropriate and in accordance with Florida law and telehealth prescribing regulations. Certain controlled substances may not be prescribed via telehealth.

Yes. All visits and communications are conducted through secure, HIPAA-compliant systems to protect patient privacy.

Appointments can be scheduled online through the booking system. Intake forms and payment must be completed prior to the visit.

ADHD evaluations may be available for adults. Treatment recommendations are individualized and may include behavioral strategies, therapy referrals, and medication when clinically appropriate.

This practice serves adolescents and adult patients (13 years and older) who are physically located in the state of Florida at the time of their visit.

United Quality Care currently operates as a cash-pay practice.
We do not bill insurance directly at this time.
Payment is due at booking.
HSA and FSA cards may be accepted when applicable.
Insurance participation may become available in the future.

This practice does not provide care for medical emergencies or high-risk conditions, including but not limited to:
Chest pain
Shortness of breath
Stroke symptoms
Severe abdominal pain
Mental health emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

Medical weight management may include prescription medications when clinically appropriate. Medication selection is based on medical history, clinical guidelines, and patient safety. Certain medications, including controlled substances, may require additional evaluation, monitoring, or in-person care. Prescriptions are not guaranteed.

When medically necessary, laboratory testing or imaging may be ordered through third-party facilities. Costs for labs or imaging are not included in visit fees and are the patient’s responsibility.

For many primary care and mental health concerns, telehealth can provide convenient and effective access to care. Some conditions may require in-person evaluation, testing, or referral.

Controlled substances are prescribed only when clinically appropriate and in compliance with Florida and federal regulations. Not all medications can be prescribed via telehealth.

If telehealth is not clinically appropriate, you will be advised to seek in-person care or referred to another provider.

When clinically appropriate and permitted by Florida law, psychiatric medications may be prescribed as part of a treatment plan.
Some medications may require additional evaluation, monitoring, or coordination of care.
