By using this website or scheduling an appointment, you agree to the following terms.

Telehealth Services

Medical services are provided via telehealth to patients physically located in the state of Florida at the time of the visit. Telehealth may not be appropriate for all medical conditions.

No Emergency Care

This practice does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

Provider Relationship

Use of this website does not establish a provider-patient relationship until an appointment is scheduled and care is formally initiated.

Prescriptions

Medications are prescribed only when clinically appropriate and in compliance with Florida law. Certain medications may not be prescribed via telehealth.

Controlled Substances

Certain medications, including controlled substances, are prescribed only when clinically appropriate and in compliance with federal and Florida state law. Telehealth prescribing of controlled substances may be limited and may require additional evaluation, documentation, or in-person assessment. Not all patients will qualify.

Payment

This is a cash-pay practice. Payment is required at the time of booking. Insurance is not billed.

Cancellations & No-Shows

Appointments may be subject to cancellation or no-show fees as outlined during booking.

Limitation of Liability

Medical decisions are made based on information provided by the patient. This practice is not responsible for outcomes resulting from incomplete or inaccurate information.

Governing Law

These terms are governed by the laws of the State of Florida.
